Orthopedic Rehabilitative Services

Diabetes Care Integrated Services
Millions of Americans experience difficulties associated with Type I or Type II diabetes. Being able to remain at home can be challenging in learning how to perform monitoring and self-care. Homestead emphasizes patient instruction and close monitoring that includes the entire health team.
Therapy Services Available
Homestead offers Physical Therapy, Occupational Therapy, and Speech Therapy as ordered by your Physician. These services are usually covered by Medicare and/or Insurance.
Geriatric Wellness Program
"Old" is a state of mind and Homestead is uniquely suited to assessing and expanding the abilities of older patients so that they enjoy the full extent of those later years. We perform a thorough assessment of the patient's mental and physical status and evaluate their home setting for safety and supportive equipment and design.

How do I get your agency services?
Just call us for information at 806-785-4540.
Office hours are from 8:00 a.m. to 5:00 p.m. daily, Monday through Friday. After regular office hours we have a designated staff nurse who responds to all emergency calls.
We will work to maintain communication with you regarding your care. A large number of telephone messages are handled each and every day. Should you call, we ask that you give all necessary information to the receptionist or nurse. In this manner, your problem can be relayed promptly to the doctor for action. Office personnel will relay results of tests and answer questions as per the doctor's instructions.
Our telephones are answered 24 hours a day including weekends and holidays. If you call after our scheduled office hours, the telephone will relay your message to a beeper carried by the nurse on-duty.

Making Contact
Home health referrals can be initiated by the physician, the patient, or their families. Upon receiving the referral request we will verify the patient's eligibility and appropriateness for home health services. We will contact the attending physician for orders and to generate the initial treatment plan.
Medicare pays 100% of the expense of our services in the home if the patient needs skilled nursing care or rehabilitative therapy, the patient is homebound, and the service has been ordered by the physician. Qualifications under which other health insurance carriers operate can vary greatly. We will check with your insurance carrier and advise you of your coverage.
