AAA > 2-1-1

2-1-1 Logo2-1-1 is the new abbreviated dialing code for free information and referrals about community organizations and resources. It serves as the first number to call to link individuals and families to appropriate resources and information. 2-1-1 is available 24 hours a day, 7 days a week in multiple languages. If you experience problems dialing 211, please call 806-762-5043.

People calling 2-1-1 in Texas are connected directly to the 2-1-1 Area Information Center (AIC) for their community. The AIC maintains a comprehensive regional database of resources including federal, state, and local government agencies, community based organizations, and qualified private businesses.

2-1-1 Texas is a program of the Health and Human Services Commission and the City of Lubbock Community Services Block Grant Program, in collaboration with South Plains Association of Governments.
 

AAA > Information & Assistance

The comprehensive Information and Assistance project is the central component of SPAG's access and assistance delivery system. It offers information to seniors on all aspects of aging services including housing, income assistance, employment and volunteer opportunities, consumer affairs, transportation, social programs and benefits/legal services. SPAG's public awareness campaign in English and Spanish includes contact with the media, speaking engagements, public service announcements, promotional materials, brochures, and TV and radio interviews. You can also receive information referral from 2-1-1.

AAA > Legal & Benefits Counseling

Through staff and trained volunteers, SPAG provides legal/benefits counseling assistance on such matters as wills and estates, living wills, durable power of attorney for health care, and property disputes. Areas of service are listed below.

Assistance is also available with entitlement benefits and in obtaining legal counsel for those who cannot afford to hire an attorney. Benefits counseling includes assistance with Medicare claims and appeals, Medicare supplement insurance, Social Security, Medicaid, Food Stamps, disability, and long term care insurance. Bilingual volunteers assist elderly clients with document preparation.

AAA > Care Coordination

The Care Coordination program provides assistance to senior citizens 60 years and older who are in need of services that include minor residential repair, emergency response systems, eye exams and eyeglasses, hearing aids, prescription medications, dental services and durable medical equipment such as walkers, and lift chairs. Additional programs include Homemaker Services, Personal Assistance, Adult Day, Chore Maintenance, and Medical transportation. The purpose is to promote healthy independence at home.

The Care Coordination program also implements Resources United meetings that are held every 2nd Thursday of each month. Meetings are held to provide presentations and case referrals between local organizations about various programs and services available to people in the region. It is a volunteer consortium of more than 75 social service and healthcare organizations, faith-based and other community groups, and governmental.

AAA > Ombudsman Program

The Ombudsman program serves as an advocate for seniors' resident rights in long-term care facilities such as assisted living and nursing homes. Ombudsmen work in partnership with nursing home residents, staff, and families to ensure quality of care and quality of life for long-term care residents. Ombudsmen work in partnership with the Texas Department of Aging and Disability Services Long-Term Care Regulatory Division. 

Certified SPAG staff and volunteer ombudsmen visit 36 nursing facilities and 44 assisted living facilities on a regular basis as well as participate in open hearings, family and resident council meetings, unannounced inspections, complaint investigations, and annual surveys. The program also assists residents and families with nursing home placements, questions regarding nursing home life, finances, spousal impoverishment, and Medicare/Medicaid eligibility.

AAA > Family Caregiver Support Program

The Family Caregiver Support Program is designed to provide assistance, support, and services - such as respite, emergency response systems, or other supplemental services - to caregivers. Caregivers need support in the everyday activities that surround the long-term care of their loved one. This way, caregivers can retain greater control over their personal life and health. The focus is to empower the caregiver with training, education, information, support groups, and temporary relief from care giving responsibilities.

To qualify for this program, the caregiver or care recipient must be 60 years old or older, or they must be custodial grandparents of children less than 18 years of age. Caregivers may be any of the following: spouse, parent, sibling, grandparent, friend, neighbor, any relative, or anyone providing care for a loved one.

AAA > Senior Citizen Centers

Senior citizens who live on the South Plains, especially those in rural areas who live alone and are poor, rely on their local senior citizens centers to provide critical services such as nutrition, transportation, telephone reassurance, and health screenings. Senior citizen centers are the focal points for delivery of many aging services. Some of the services offered are meals, recreation and social activities, nutrition/health education, exercise programs, transportation, and volunteer opportunities. Congregate and home delivered meals are at the core of many of the multipurpose senior citizen centers. Of the 23 senior centers, 22 are also nutrition sites offering hot noon meals during the week. Meals must meet at least one-third the Recommended Dietary Allowance (RDA). SPAG contracts with a registered dietician who plans menus for nutrition service providers, evaluates their projects, and conducts nutrition education lessons, which are passed on to participants at the senior centers and recipients of home delivered meals.

Anonymous donations are encouraged to help defray costs; however, senior citizens will not be denied services because they cannot or choose not to contribute. The centers also serve as the locations for the caregiver resource libraries which provide valuable information for caregivers and care receivers.
