Home Health services came about as there was a
recognized need to attend to the medical needs of people outside
of the hospital setting but beyond the ability of friends and
family to provide it.

* Your physician has evaluated your case and determined
that you need it
* You would benefit from services such as skilled
nursing, rehabilitative therapies

How are these services paid for?
Home health services are covered by your
Medicare benefits. Most other health insurance carriers provide
some level of coverage for such services and we will contact the
carrier and advise you of your plan. We understand that dealing
with insurance can be confusing and we partner with you to help
you understand and determine what is right for you or your loved
ones.
What about medical equipment such as
walkers, wheelchairs, or medical supplies?
We work with established durable medical
equipment suppliers when these items are needed in your care.
Medicare, Medicaid, and other health insurance offer coverage
for such equipment and we will determine what your benefits are
in obtaining such items.

Who can make the referral to your agency?
Referrals to our agency can be made by completing the on-line
referral form which is available by Clicking Here. Referrals are
usually received by:
*Physician
*Social Workers
*Health Care Facilities
*The Patient and Family

for orders and management planning.
