Personal visit, written plan and family
session to address needed care services, placement options,
professional referrals and resources.

Evaluation and recommendations following
personal visit to determine the most suitable placement.
Who Uses Our
Program
Anyone who is involved with
the care of an older adult may seek out our service. Our
Care Manager does not replace family involvement but
acts in a manner to help gather information for decisions and
carrying out action plans. Care managers may be hired by:
*Spouses
*Children or other Family Members who may live some
distance from the client
*Guardians or conservators
*Attorneys
*Trust Directors
A Guide Along
the Way
As we age we may become overwhelmed by the changes that take
place. With families spread across the country there may be
concerns about how well a parent or sibling might be managing a
situation. Homestead Care Management is designed to
provide services in a changing, evolving world that identifies
the specific need and then provides the targeted service.
Matching the Moment, Looking
Toward the Future
Our Care Manager helps
explore all future care options based on the older adult's and
family's desires. The plan is updated quarterly and, should the
need arise, the Manager is ready to step in ... with the
plan the client set up!
Safety Consultation
Our Care Manager
conducts a thorough safety assessment in the home of the older
adult, and makes recommendations about ways to make the home
safer. The Manager can also assist with identifying community
vendors that provide necessary repairs or modifications.
Vacation Respite
Care management (and caregiving if needed) in
the primary caretaker's absence. Oversight and regular
check-ins, a backup plan in case of an emergency, and a
Caregiver who can step into a situation for a short period and
care for the older adult.
Support and Respite
Four-hour weekly caregiving respite so that
caregivers can take a break.
At-Home Assisted Living
A plan tailored to meet the needs and desires
of all involved to help the older adult continue living in
his/her own home.
Bathing Assistance
Following a no-cost evaluation by our
Care Manager, this service provides
assistance with all of your bathing needs.
Money Management
Ongoing assistance with daily money management
including timely bill paying and detailed accounting for taxes.
This can be done in the Homestead office or in the client's
home.
Management of Legal Needs
Our Care Manager will talk to you,
understand your needs and future desires, and assist you in
finding an attorney that is suitable in preparing the legal
documents that will identify your
health care decisions that respect your wishes and honor your
values.
Service Payment
"Assist Life" Care Management
services are based on a fee-for-service arrangement. We do not
receive payment from Medicare or Medicaid and we do not take
referral fees from any of the professionals that we may list in
our directories of providers that are given to our clients. We
do not accept such fees so that you know that we are unbiased in
our review of local providers and institutions selecting only
what is best for the client.
Service Fees
Initial Assessment....
$750.00
Includes an initial in-home consultation with our
Care Manager, an inspection for environmental safety,
development of an emergency plan, organization of health
information and creation of record library, and any follow-up or
further investigation to complete the written report. We then
will meet with the client and family as desired to go over the
report and recommendations.
In-Office Assessment....
$200.00
Includes a 1.5 hour interview with the client by the
Care Manager held at our office. Information is
obtained completing a medical and social history so that client
needs can be identified. Specific information is provided
concerning what action is needed and what strategies are best
suited to obtain these goals.
