In addition to the provision
of health care services, all hourly care patients are managed by one
of the Home Care Coordinators, who monitor the care of each patient.
In order to provide high quality care that simplifies the situation
for the patient
our coordinators can manage and administer their
care by providing physicians and family members with periodic
updates, ordering refills, and identifying/assisting the patient and
their family in solving problems that may arise at the patient's
home.
