Medical Social Work ServicesEvery

patients take back control of their lives. Our
social workers serve as advocates in assisting patients and their
families with the many emotional and social issues that
can develop as the person is recovering in the home.
Working from a knowledgeable understanding of what community
resources are available they identify what other resources
can be brought to bear for the benefit of the patient.
Homestead recognizes the need for an holistic treatment
plan involving social, emotional
and physical health for successful healing. Our
social worker consultants make it their job to
cultivate a network of connections to community
resources, helping patients and families develop
healthier coping methods, gain access to community
resources, evaluate long-term care options, and
determine their eligibility for local, state and
federally-assisted programs.

- Assessment of patient's
social/emotional needs
- Community resource planning
- Counseling for long-term care
planning, including advanced directives, powers of
attorney, living wills and resuscitation
instructions

workers coordinate with other agency providers and the physician
