"Weigh to Health" Program Components
The route to a healthy lifestyle is not a
secret but achieving a balance between the factors of genetics,
nutrition, exercise, physical and emotional health is not easy.
Offered by our Medical Director Chris Rose, M.D. the program came about based on a recognized need to
offer integrated health care. The professional staff is made up
of different health care specialists who are experts in their
fields. Using research-based methodology our programs reflect
the latest in contemporary lifestyle improvement strategies.
"Weigh to Health"
Program Objectives
The objectives of the
"Weigh to Health" Program are to:
1. provide
a thorough assessment of the current health status of the
participant, including both physical as well as psychological
functioning.
2. identify
risk factors for the participant and offer recommendations for
reducing or eliminating these.
3. offer
interventional services to reduce or eliminate identified risk
factors. Programs will include:
a.
Weight Reduction Services: To help
participants achieve an optimal body weight and maintain this
value over time
b.
Performance Enhancement Services:
To help participants improve in targeted performance areas
including physical, sexual, and emotional functioning.
The program will offer
assessment services that can be provided alone or bundled with
the interventional components.

Interventional
Services
A time-limited program
of 12 weeks will be offered. This enables participants to pace
themselves and yet have a definitive “end-point” to their
program. The number of participants will not exceed 12 people in
any scheduled class. Classes will meet on a weekly basis and
last for approximately 90 minutes. Participants will be provided
a program manual and enrolled in the on-line fitness tracking
program. They will return their participant cards each week or
go on-line to complete and submit the requested information.
Nutritional Component
As a function of the
initial program assessment the participant will be provided with
a detailed evaluation of their present physical functioning and
risk factors associated with being over-weight. A customized
weight reduction program will be included in the report provided
to the patient. This program will include information on proper
dieting and suggested target goals and time lines for their
attainment.
Medication Component
Based on the initial
assessment Dr. Rose will provide the participant with
prescription medication to facilitate weight reduction. Patients
will return at monthly interval to gauge their progress and to
monitor their health status in relation to the medication.
Exercise Component
The initial assessment
report will contain information on how to improve physical
activity with specific recommendations for the participant. A
pedometer will be given to the participant to provide
performance data during the twelve week program. In addition the
participant will be provided a one year membership to a local
exercise facility.
Psychological Component
Classes in behavior
modification and cognitive re-structuring will be made available
to the participant.
Our program works to
create a partnership with our patients in helping them create
change within their normal home and work environments. Services
include confidential physician visits, medication, behavioral
health and educational services and written guides to help
answer your questions and to guide your actions toward success.

Initial Evaluation

Coming into the program we will want to
know a great deal about you and unique aspects of your functioning.
One of our principle tools in the state-of-the-art Tanita Body
Composition Scale. Using Bioelectric Impedance Analysis (BIA), the
scale provides an exact measurement of:
BMI - Body Mass Index (Weight in kg/Height in m2)
BMR - Basal Metabolic Rate (The total energy
expended
by the body to maintain normal functions)
FAT PERCENTAGE - Percent of body weight that is
adipose tissue
FFM - Fat Free Mass
TBW - Total Body Weight (50%-70% of total body
weight)
FAT MASS IMPEDANCE - The body's resistance to
electrical current. Muscle is a conductor and adipose
tissue is a resistor, so as your muscle increases your
impedance will be lower.
DESIRABLE RANGE - Healthy range for Fat % and Fat
Mass
During the
initial evaluation you will undergo a cardio-vascular screening
which will include EKG. We also urge all new patients to have lab
work performed so that we have precise metabolic functioning
information. The physician will review the results of this testing
and the patient's medical history to design an appropriate weight
loss program.
Program Fees and Reimbursement
Because of the intensity of
the program it is difficult to obtain reimbursement from traditional
health insurance related to cost-containment that is short-sighted
and, in the end, more costly to consumers in both financial expense
and personal health. We will provide you with a summary list of
program components and procedural codes so far as they exist and you
may seek reimbursement from your insurance.

A Program for You
Following the initial screenings, meeting with the team physician
and other professionals, you will receive a comprehensive lifestyle
report. This report will give you an assessment of your current
functioning and risk factors that you should be aware of. It will
also outline reasonable treatment goals that are broken down into
manageable steps. Recommendations for nutrition and medication are
combined with suggested focused interventions for reduced stress and
enhanced functioning.
Program participants are
seen monthly by the medical team in regular clinic visits. In
addition there are scheduled group sessions that offer a variety of
topics and you are free to choose which you want to attend. We do
request that you make reservations for these group classes so that
we can keep the number of people attending to a low number and
ensure quality attention from the instructor.
At interval your
health status will be re-assessed using the BIA instrument and other
measures. Regular meetings with our medical team will facilitate the
achievement of optimal success and balance in your lifestyle.
